Birth Certificates

Florida's Department of health and rehabilitative services carries vital records dating back to April 1865 and many death vital records from August 1877 and on.  Most of the vital records date back to January 1917.  If you do not know the exact date you are looking for then there will be a $9.00 fee on birth certificates and a $5.00 fee for death certificates for the first year searched and a $2.00 fee for each additional year up to a maximum of $50. Any additional copies you request or $4.00 each when the request is made at the same time of the original order.  All fees do include one certification of record (if found) or a certified statement informing you that the record was not on file.

Please make your personal check or money order payable to Office of Vital Statistics. Florida's Department of Health and Rehabilitative Services does accept personal checks. You can confirm the current fees by listening to the recorded message at (904) 359-6900.


Florida Death Records Information

The state office in Florida carries vital records dating back to April 1865.  Florida also carries vital death records dating back to August 1877.  Most of the vital records began in January 1970.  If you do not know the exact date you are looking for on a birth certificate then the fee is $9.00, five dollars for death certificates for the first year searched, and two dollars for every additional year are up to $50.  The fee does include one certification of record (if found) or a certified statement from the state office stating that the record was not on file.  If you request additional copies at the same time then the fee is $4.00.
