The Illinois Department of Public Health carry vital records dating back to January 1960.  If you will require records earlier than 1916, please contact the county clerk in the county in which the event occurred (fees vary throughout different counties). A search of state files costs $10 and if the record is not found then one certificate is granted at no additional cost. If you require additional certificates at the same time of order then the fee is $2.00 each.  If you require a full certified copy then the fee is $15 each and any additional certified copies that are ordered at the same time will cost $2.00 each.
